Key Responsibilities:
- Personal Care & Daily Living Support: Assist individuals with activities of daily living (ADLs) including personal hygiene, dressing, grooming, meal preparation, and medication reminders (after proper training).
- Skill Development & Independence: Implement individualized support plans to teach and reinforce independent living skills, such as money management, household chores, and healthy lifestyle choices.
- Community Engagement: Facilitate participation in community activities, social events, recreational outings, and appointments, promoting integration and social inclusion.
- Health & Safety Monitoring: Observe and document changes in physical, mental, or emotional health; ensure a safe and clean living environment; and respond appropriately to emergencies.
- Behavioral Support: Adhere to and implement positive behavior support plans, utilizing de-escalation techniques as needed (with training), to promote a stable and supportive environment.
- Transportation: Provide safe transportation to appointments, community events, and errands using personal or company vehicles as required.
- Documentation: Maintain accurate, timely, and thorough documentation of daily activities, progress notes, incident reports, and medication logs.
- Advocacy & Communication: Act as an advocate for individuals, respecting their choices and preferences, and communicate effectively with family members, guardians, and the support team.
Qualifications:
- Education: High School Diploma or GED required.
- Experience: Previous experience as a DSP, caregiver, CNA, HHA, or in a similar support role is preferred, but passionate individuals with a willingness to learn and train are strongly encouraged to apply.
- Age: Must be 18 years of age or older.
- Driver's License: Valid driver's license and reliable transportation are essential.
- Background Checks: Ability to pass criminal background checks (state and federal) and drug screening.
- Health Requirements: Ability to pass a physical examination and TB test.
